Mandated Reporting Form
As a Mandated Reporter under Alabama law, I am required to report the following:
Child abuse as defined by Alabama law:
Under Alabama law, it is "harm or threatened harm to a child's health or welfare which can occur through nonaccidental physical or mental injury; sexual abuse or attempted sexual abuse; sexual exploitation or attempted sexual exploitation."
Child neglect as defined by Alabama law:
Under Alabama law, it is "negligent treatment or maltreatment of a child, including the failure to provide adequate food, medical treatment, clothing, or shelter: provided, however, that a parent or guardian legitimately practicing his religious beliefs who thereby does not provide specified medical treatment for a child, for that reason alone shall not be considered a negligent parent or guardian; however, such an exception shall not preclude a court from ordering that medical services be provided to the child, where his health requires it."
Child Pornography as defined by Federal law:
Child pornography is a form of child sexual exploitation. Federal law defines child pornography as any visual depiction of sexually explicit conduct involving a minor (persons less than 18 years old). Images of child pornography are also referred to as child sexual abuse images. Federal law prohibits the production, distribution, importation, reception, or possession of any image of child pornography. A violation of federal child pornography laws is a serious crime, and convicted offenders face fines severe statutory penalties.
Adult Abuse as defined by Alabama law:
The Alabama Adult Protective Services (APS) Act of 1976 addresses abuse, neglect, and exploitation of adults who are incapable of protecting themselves and have no one willing or able to protect them. It outlines the responsibilities of the Department of Human Resources, law enforcement, caregivers, individuals, and agencies in reporting and/or investigating these cases.
Threat of Harm to self or others:
Situations involving risk of harm to self (including suicidal thoughts or intent) or harm to others (including ideation about harm).
